EMROI was developed by Dr. Syed T. Raza, a practicing cardiac surgeon at Columbia University Irving Medical Center, after years of seeing the same documentation problem firsthand.
Instead of dictating a free-text note, the surgeon answers procedure-specific structured questions. EMROI uses those answers to generate a procedure-accurate anatomical illustration and assemble a complete operative note.
The result is a standardized, illustrated record of the operation — created at the point of care and delivered automatically to the people who need it.

Dr. Raza built EMROI because he was experiencing the documentation problem firsthand — inside the OR, every single day. Every surgeon documented differently. Every note required interpretation. The care chain downstream was left piecing together what happened from dense, inconsistent free-text.
EMROI began as a solution to his own documentation problem — and has since evolved into a platform serving the entire cardiac care chain at one of the world’s leading cardiac surgery programs.
